Case Management refers
to comprehensive acts of gathering information, communication,
service arrangement and advocacy on behalf of individuals
who require assistance. It is the function performed by
“qualified mental retardation professionals (QMRPs),
on CSO, Inc.’s staff.
CSO, Inc.’s
QMRPs evaluate the needs, abilities and options of each
individual (case). Based on an analysis of each individual’s
medical, psychological, educational and social background,
QMRPs determine “appropriate” services and oversee
both the development and implementation of the individual
service plan (ISP).

of case management services include:
- personalization
- services consistent with eligibility
- responsive to client base
- flexible
An individual’s
“case” is open, active and "tailored"
to address a continuum of services and needs as they arise.
QMRPs maintain contact and advocate on behalf of the individual
even after placement in services has been achieved. The
individual’s service plan is reviewed and revised
yearly or more frequently to accurately reflect the progress
and needs of the individual.
