CSO, Inc’s Intake Specialists conduct interviews
with persons seeking services.
If an individual is eligible to receive services from Illinois
Department of Human Services’ Office of Developmental
Disabilities, Intake registers the individual, creates a
“case” record and dispatches a “qualified
mental retardation professional” (QMRP) to coordinate
assessments, applications, arrangements and ongoing advocacy
on behalf of the individual.
Intake Specialists
obtain information about individuals’ service needs,
diagnoses, service history, legal status, medications, medical
treatments, household income and insurance. Using this information,
Intake assesses individual eligibility for a variety of
services. Intake informs individuals about the process to
access accommodations or refers individuals, elsewhere, to
appropriate resources.
