For TBI case management includes:
those with physical and cognitive impairments, acquired
after age 22 and resulting from accident, medical illness
or violent crime. These newly acquired disabilities
are chronic and permanent impairments affecting intellectual
and emotional functioning as well.
Case Management is responsible for helping individuals
to assess and identify ways of adapting their lives and
identifying supports that will be needed. Most often, individuals
require rehabilitative therapies, in-home support services
and ongoing advocacy to ensure safety and progress toward
rehabilitation.
CSO, Inc.’s case managers call and visit service recipients
on a monthly basis. Initially, they assess the need for
in-home supports, help develop an individualized service
plan and make service arrangements. Once the services are
in place, they continue to monitor implementation of the
plan and the delivery of services. The plan is reviewed
on a yearly basis or more frequently and revised as needed.
